Job Description
POSITION SUMMARY
The Licensed Nurse is responsible for overseeing and coordinating the health care needs of children, youth, and young adults residing at House of Serenity Residential Treatment Centers. The Nurse provides nursing assessments, medication management, health education, medical care coordination, infection control oversight, and health-related training to ensure the physical health, safety, and well-being of residents. The Nurse works collaboratively with physicians, psychiatrists, therapists, social workers, direct care staff, families, placing agencies, and community healthcare providers to deliver trauma-informed, child-centered, and medically appropriate services in accordance with Texas Residential Child-Care Licensing requirements and Texas Child-Centered Care (T3C) principles.
MINIMUM QUALIFICATIONS
Education Graduate of an accredited nursing program. Licensure Current Texas Registered Nurse (RN) License or Licensed Vocational Nurse (LVN) License in good standing. Experience Experience working with children, adolescents, behavioral health, foster care, residential treatment, pediatrics, or mental health populations preferred. Additional Requirements Current CPR and First Aid Certification. Valid Texas Driver's License and acceptable driving record. Successful completion of all HHSC background check requirements.
ESSENTIAL JOB DUTIES AND
RESPONSIBILITIESHealth Assessments and Monitoring Conduct health screenings and nursing assessments upon admission and as needed. Monitor residents' physical health and medical conditions. Review medical histories and health records. Identify health concerns requiring medical attention. Monitor residents for illness, injury, or changes in health status. Ensure residents receive appropriate medical care and treatment. Medication Management Oversee medication administration practices. Ensure medications are stored, documented, and administered according to physician orders and House of Serenity policies. Review Medication Administration Records (MARs). Monitor residents for medication side effects and adverse reactions. Coordinate prescription refills and medication orders. Conduct medication audits and inspections. Ensure compliance with controlled substance requirements. Medical Care Coordination Schedule and coordinate medical, dental, vision, psychiatric, and specialty appointments. Communicate with healthcare providers regarding resident care. Obtain and maintain physician orders. Coordinate hospitalizations, emergency room visits, and follow-up care. Ensure continuity of medical services. Assist with discharge medical planning. Infection Prevention and Control Oversee infection control practices throughout the facility. Monitor communicable disease prevention measures. Educate staff regarding infection control procedures. Ensure compliance with universal precautions. Monitor outbreaks and implement infection prevention strategies. Maintain infection control documentation. Emergency Medical Services Respond to medical emergencies. Assess emergency situations and provide nursing intervention. Coordinate emergency medical services when necessary. Provide follow-up care after medical emergencies. Participate in emergency preparedness planning. Health Education Provide health education to residents.
Educate residents regarding:
Personal hygiene Nutrition Medication compliance Wellness practices Disease prevention Healthy lifestyles Provide health-related education and training to staff. Documentation Responsibilities Maintain accurate nursing documentation. Complete health assessments and nursing notes. Document medication-related activities. Maintain medical records in accordance with licensing requirements. Ensure documentation is timely, accurate, and complete. Maintain confidentiality of health information. Staff Training Responsibilities The Nurse may provide or assist with training regarding: Medication administration; Infection control; Universal precautions; Communicable diseases; First aid procedures; Medical emergencies; Health screenings; Medication storage and disposal; Documentation requirements. Child and Family Team Participation Participate in Child and Family Team Meetings. Provide health-related recommendations. Assist with treatment planning and discharge planning. Collaborate with families and treatment team members.
T3C RESPONSIBILITIES
The Licensed Nurse shall support and promote: Youth Voice and Choice; Family Engagement; Child and Family Team Meetings; Permanency Planning; Community Integration; Trauma-Informed Care; Strength-Based Care; Individualized Service Planning; Resident health and wellness.
REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES
Knowledge of pediatric and adolescent healthcare. Knowledge of medication administration and monitoring. Knowledge of infection control principles. Knowledge of trauma-informed care. Ability to assess and respond to medical emergencies. Ability to maintain accurate documentation. Ability to train and educate staff. Ability to work collaboratively with multidisciplinary teams. Strong communication and organizational skills.
SUPERVISORY RESPONSIBILITIES
The Licensed Nurse may: Monitor medication administration practices. Provide guidance to Direct Care Staff regarding health-related matters. Assist in evaluating compliance with medical and nursing policies. Participate in quality improvement activities.
WORKING CONDITIONS
Residential treatment environment. Frequent interaction with children, youth, young adults, families, and healthcare providers. Exposure to communicable illnesses and behavioral health crises. May require evening, weekend, holiday, or on-call availability.
PHYSICAL REQUIREMENTS
Ability to stand, walk, bend, lift, and move throughout the facility. Ability to respond quickly to emergencies. Ability to transport residents when necessary. Ability to perform nursing duties safely and effectively.

Pay:
$39.32 - $47.35 per hour
