Responsibilities Conduct comprehensive patient assessments, including vital signs, physical examinations, and physiological evaluations to determine individual care needs. Administer medications and injections with precision, including IV infusions, catheterizations, and aseptic techniques to ensure safety and efficacy. Manage complex clinical procedures such as wound suturing, ostomy care, ventilator management, and dialysis treatments in home settings. Utilize EMR/EHR systems like Epic or Cerner to document medical records accurately, ensuring compliance with HIPAA regulations and NCQA standards. Provide specialized care for diverse populations including geriatrics, pediatrics, individuals with developmental disabilities, memory care patients, and those requiring hospice or palliative medicine. Coordinate discharge planning and case management activities to facilitate smooth transitions between healthcare settings. Respond promptly to urgent triage calls and critical situations such as acute pain episodes or trauma incidents, applying trauma medicine protocols when necessary. Experience Proven nursing experience in hospital medicine, ICU (Intensive Care Unit), or Level I/II trauma centers with exposure to emergency medicine or critical care response procedures. Hands-on experience with advanced clinical procedures such as cardiac catheterization, laboratory specimen collection/processing, and infectious disease care. Demonstrated ability to work effectively in diverse settings including nursing homes, assisted living facilities, outpatient clinics, or home health environments. Familiarity with EMR/EHR systems like Athenahealth or eClinicalWorks; proficiency in

ICD-9/10

coding for accurate documentation. Strong background in managing complex cases involving pain management, occupational health issues like Workers' Compensation law compliance, and post-acute care planning. Experience working with individuals with disabilities or behavioral health needs; knowledge of dementia and Alzheimer's care is highly valued. Ability to perform technical skills such as phlebotomy, venipuncture, laboratory testing procedures, tube feeding management, and sterile processing techniques.
