- Rivendell Behavioral Health Services is an 80-bed full-service behavioral healthcare acute care facility located in Benton, AR, that has been providing quality health care to the residents of Arkansas since 1985.
ESSENTIAL JOB DUTIES/RESPONSIBILITIES 1.
Performs ongoing assessments of the behavioral and medical needs of the patients each shift. Charts those observations of the patient in the nurse's note each shift, in a legible and concise manner. 2. Carries out physician orders regarding diet, medication and treatment. 3. Performs Nurse Observation rounds as needed or directed per hospital policy. 4. Under the direction of the Charge Nurse authorizes emergency implementation of seclusion and physical restraint in the physician's absence according to hospital policy. Participates in, supervises and documents, special treatment procedures, progressive interventions and de-escalation of patients according to hospital policy. 5. Reviews all EIR documentation, and initials all entries made by the MHA/LPN or other staff. 6. Assists in coordinating treatment as a team member through participation in treatment planning meetings. 7. Educates patient/family toward an understanding of health/illness and the plans for post discharge care. 8. Performs basic first aid, CPR as needed and manages emergency situations as necessary. 9. Administers medications to patients and completes appropriate documentation on the MAR. 10.In addition:
if working the 11/7 shift, reviews charts nightly ensuring that all orders are on the MAR, completes night chart audits on all new admissions and programs to program transfers and participates in PI programs. 11 Ability to demonstrate knowledge of developmental tasks of children and adolescents. 12 Ability to demonstrate knowledge of developmental tasks of adults. 13. Intervenes appropriately to prevent escalation of patient behavior; uses effective verbal de-escalation. 14. Thoroughly documents patient information, observations, changes in the patient's condition and nursing care provided.
