Shift:

Monday - Friday, 8-hour shift starting at 9:00 AM ET / 8:00 AM CT Position Summary We are seeking a highly skilled and empathetic Registered Nurse to deliver telephonic Medication Therapy Management (MTM). In this role, you will directly improve patient care by conducting telephonic medication reconciliations with high-risk members, identifying medication-related problems, and coordinating resolutions. You will interact daily with patients, prescribers, caregivers, and internal resources to develop member-specific treatment plans based on nationally recognized guidelines. Using innovative behavioral modification and motivational interviewing techniques, you will empower patients to achieve their health goals and guide them on their path to better health. Key Responsibilities Performprehensive, telephonic medication reviews and reconciliations for high-risk patients. Identify, document, and resolveplex medication-related problems. Communicate clinical rmendations clearly and professionally to prescribers, caregivers, and patients (both verbally and in writing). Utilize motivational interviewing and behavioral modification skills to drive positive health oues. Maintain accurate, real-time documentation within multiple internal systems while managing inbound and outbound calls. Position Requirements

Licensure & Geography:

Must hold an active, unrestricted RN license in good standing. Must reside in and hold proof of a participating Compact Licensure state.

Home Office & Technical Infrastructure:

A dedicated, quiet workspace free from distractions to ensure strict adnce to HIPAA guidelines. A hard-wired internet connection is mandatory. Your workspace must allow you to plug directly into your modem via an Ethernet cable (wireless/Wi-Fi is not permitted). Candidates must provide their own Ethernet cord and surge protector. MTM Delivery Experience:

Strong hands-on experience delivering telephonicprehensive medication reviews, identifying drug-related problems, andmunicating clinical rmendations to prescribers.

Call Center & Telephonic Engagement:

Comfort handling back-to-back inbound/outbound calls, strong multitasking abilities, and experience with motivational interviewing.

Clinical Knowledge:

Strong foundation in chronic disease state management (certifications such as Certified Diabetes Care and Education Specialist are a plus). Click on now for instant consideration!
