Job Description

Job Overview We are seeking a compassionate a Registered Nurse (RN) Case Manager to join our In Home Care team. The ideal candidate will provide high-quality patient care in various settings, including assisted living facilities, clinics, and in private homes. Responsibilities Meet with Veteran Patients to establish care, complete detailed assessment and communicate well with families, Caregiving Team and VA. Complete and maintain all files necessary for compliance. Monitor patients' vital signs and report any changes to the Client Care Coordinator, family and VA. Provide support in travel nursing assignments as needed, adapting to various healthcare environments. Maintain accurate patient records and documentation in compliance with healthcare regulations. Collaborate with healthcare team members to develop and implement patient care plans. Educate patients and their families on health management and wellness practices. Supervise and train Caregiver Teams to ensure proper care and standards are met. Qualified RN to oversee CNA testing of Caregiver Team. Requirements Valid RN license in the state of practice. Experience in assisted living, clinic settings, or critical care preferred. Excellent communication skills, both verbal and written. Compassionate demeanor with a strong commitment to patient care.
