Join our growing Team Select Family and discover a dynamic company where you're able to make a positive impact. Team Select is looking for a RN Case Manager to join our Anderson team. The RN Case Manager will split their time in the office as well in the field with the nurses and our patients. In this role, you will direct care planning and collaboration efforts for your patients to ensure a comprehensive approach to home care. The RN Case manager will report to the Director of Clinical Services/Nursing Supervisor. Responsibilities Develop the plan of care and necessary revisions. Initiate appropriate preventive and rehabilitative nursing procedures. Coordinate services with other healthcare providers. Inform physician and other personnel of changes in the patient's condition and needs. Counsel the patient and family/significant others in meeting nursing and related needs. Participate in in-service programs, supervising and teaching other home health personnel. Complete admission, re-certification and discharge assessments. Determine the amount and type of homecare needed by each patient. Refer to Physical Therapist, Speech Language Pathologist, Occupational Therapist and Medical Social Worker regarding patients requiring their specialized skills. Conduct patient care conferences. Attend staff meetings as scheduled. Complete documentation and paperwork promptly per agency policy. Immediately reports to Director of Clinical Services/Nursing Supervisor any patient incidents/variances or complaints. Perform other tasks as assigned. This is a Monday - Friday daytime position with the flexibility to give you a great work-life balance Qualifications Graduate of an approved school of professional nursing and licensed in the state(s) in which practicing One (1) year nursing experience Comfortable with Pediatrics and Geriatrics Maintain current CPR certification.
