Position Summary The Wellness Nurse (LVN) is responsible for coordinating resident healthcare services and medication management under the direction of the Health Services Director and attending physicians. This position promotes resident independence while ensuring that all clinical services are delivered safely, compassionately, and in compliance with applicable federal, state, and company regulations. The Wellness Nurse serves as an essential clinical resource for residents, families, physicians, and community staff. Key Responsibilities Coordinate and oversee resident healthcare needs in collaboration with physicians, specialists, pharmacies, therapists, and other healthcare providers. Administer medications and treatments in accordance with physician orders, the California Nurse Practice Act, and established nursing standards. Monitor residents' physical, emotional, and cognitive health, identifying changes in condition and initiating appropriate interventions. Assist with resident assessments, care planning, admissions, transfers, and ongoing evaluations. Maintain accurate, timely, and confidential clinical documentation in electronic and paper medical records. Review medication orders for accuracy and coordinate prescription refills with pharmacies and physician offices. Support and provide clinical guidance to Medication Technicians and caregiving staff regarding medication administration and resident care. Participate in resident care conferences and communicate health updates to residents, families, physicians, and interdisciplinary team members. Observe infection prevention and control practices while promoting a safe and healthy living environment. Respond promptly to medical emergencies and coordinate emergency services when necessary. Monitor incident reports, follow up on resident concerns, and assist with quality improvement initiatives. Ensure compliance with state licensing regulations, company policies, and resident rights. Participate in shift reports and communicate resident care updates to incoming staff. Assist with training and mentoring caregivers on clinical procedures, documentation standards, infection control, and resident safety. Foster positive relationships with residents and their families through compassionate communication and exceptional customer service. Perform additional clinical and administrative duties as assigned by the Health Services Director. Required Qualifications Current and unrestricted California Licensed Vocational Nurse (LVN) license in good standing. Graduate of an accredited vocational nursing program. Current CPR and First Aid certification or ability to obtain upon hire. Working knowledge of medication administration, nursing assessments, and documentation practices. Strong understanding of infection prevention, resident safety, and healthcare regulations. Excellent communication, interpersonal, and organizational skills. Ability to work independently while collaborating effectively within an interdisciplinary team. Basic computer proficiency, including Microsoft Office and electronic health record systems. Ability to successfully complete required background screening, physical examination, and tuberculosis testing. Preferred Qualifications One or more years of nursing experience in assisted living, memory care, long-term care, rehabilitation, or geriatric nursing. Experience working with residents living with Alzheimer's disease or other forms of dementia. Familiarity with California Residential Care Facility for the Elderly (RCFE) regulations. Experience supervising Medication Technicians or direct care staff. Physical Requirements Ability to stand, walk, bend, reach, and move throughout the community for extended periods. Ability to lift, transfer, push, or pull up to 50 pounds with or without reasonable accommodation. Ability to respond quickly to resident emergencies. Manual dexterity sufficient to administer medications and operate standard medical and office equipment.
