Position Overview In this Home Health Nursing role, you will provide skilled in-home nursing care to patients across Phoenix, Arizona, supporting safe recovery, comfort, and independence. You will collaborate with physicians and the interdisciplinary team to implement and update individualized plans of care, monitor patient status, and escalate changes in condition in a timely, clinically appropriate manner. Accurate, compliant clinical documentation is essential, using a fully remote workflow to ensure continuity across settings. You will also deliver clear patient and caregiver education on medications, symptom management, and home safety to reinforce adherence and reduce risk.

Required qualifications/skills:
