SCOPE AND RESPONSIBILITIES
Conduct interviews with individuals who have sustained work-related injuries, including head trauma and spinal-cord injuries. Coordinate medical/psychological care by developing, initiating, and instituting a rehabilitation plan for the disabled individual, which may include addressing safety issues. Periodically review all plans for effectiveness to assure that desired outcomes and goals are met Address case staffing with other relevant rehabilitation professionals in order to provide appropriate and adequate physical care for the injured individual as prescribed by the physician. Monitor care being provided and obtain medical opinions when/where indicated to obtain necessary medical clarification and apprise involved parties of medical status and care being rendered. Facilitate case management activities leading to vocational readiness, and establishing a return-to-work date. Provide assistance for home modification, vehicle modifications, PT/OT services, medication regimen, procuring orthotic devices, prescribed healthcare equipment, and coordinating home healthcare services, as may be required. Perform employer interviews with employer representatives to ascertain relevant job duties and essential functions of pre-injury jobs; may assist in job restructuring; determine the availability of alternate, modified, and light duty positions with pre-injury employers, as specified by the physician; conduct job analysis, as appropriate. Act as liaison, coordinating efforts with the injured individual, physicians, medical and psychological care providers, attorneys, employers and insurance professionals. Assist insurance professionals in determining and projecting future care, needs and expenses; provide pertinent information to assist life care plan specialists. Adhere to the policies and standards set forth by our URAC accreditation as well as jurisdictional legislative and regulatory requirements. May serve on the QI Committee to identify areas of improvement and future quality improvement initiatives. May attend meetings with various Company departments to coordinate case management efforts. Reports directly to a Company medical services supervisor.
EXPECTED PROFESSIONAL COMPETENCIES
Analyze current health status of injured workers to determine nursing care needs. Professional verbal and written communication skills. Internet browsing/navigation, email and basic office software competency. Understanding of procedural and legislative rules and regulations as they pertain to case management in the prevailing jurisdiction (training in this competency provided to the chosen applicant).
QUALIFICATIONS
Applicant must possess a current and unrestricted RN license in the state/jurisdiction in which the applicant is being considered for hire. One year of direct clinical care experience The following are not required; however, individuals possessing any of the following will received preference: Certification as a case manager (CCM, CDMS, CRC) Bachelor's Degree (or higher) in a health-related field and/or licensed or certified as a health care professional (in addition to RN).
