Give care in a manner that promotes quality, comfort, care, and safety for the patient and their family and/or caretaker at every visit. Make initial and ongoing assessments of the patient/family needs and carries out interventions in accordance with National Association of Hospice and Palliative Care Standards, the interdisciplinary plan of care and agency policy. Completes all necessary admission forms and other forms required for each visit with a patient. Reports to and is directed by a Hospice Clinical Manager.
Education and Experience:
Graduate of a school of nursing accredited by

Either from a Baccalaureate or Master's degree program in Nursing, or; From a state approved diploma or Associate's degree program in Nursing Must have a minimum of 1 year of experience, preferably in hospice or home care. Completion of Hospice Staff Training in lieu of hospice experience. Registered Nurse licensed in the State of Rhode Island.
