Key Responsibilities:

Perform comprehensive home health risk assessments with each visit averaging 45 minutes Evaluate patients medical and medication histories, identifying chronic and acute concerns Deliver clear education to members about their health conditions and medications Pinpoint gaps in care and provide recommendations to address these needs Document findings thoroughly; no need to create treatment plans or prescribe medications

Qualifications:

Active Nurse Practitioner license in Rhode Island Current CPR certification (BLS) Board certification through AANP or ANCC Minimum of 6 months of Nurse Practitioner experience preferred MSN degree from an accredited nursing program In this role, you will be working within Providence and surrounding communities including Woonsocket, Scituate, and Cranstontravel within these areas is supported by a mileage stipend. For your time and expertise, you will receive $80-$90 per evaluation, plus compensation for administrative time and mileage expenses.
