Responsibilities:

Conduct comprehensive home health risk assessments during visits averaging 45 minutes each Evaluate patients current and prior medical history, including conditions and medications Provide clear, concise education regarding chronic conditions and medication regimens Identify care gaps and suggest actionable recommendations for follow-up No prescribing or treatment plan development required

Qualifications:

Current Nurse Practitioner licensure in Rhode Island Active board certification through AANP or ANCC Valid CPR certification (BLS) MSN degree from an accredited nursing program At least 6 months of recent NP experience is preferred Strong communication and patient education skills
