Seeking a Nurse Practitioner for specialized home health risk assessment visits. This per diem opportunity is ideal for experienced NPs interested in flexible, self-managed scheduling and meaningful patient interactions. Shifts are available throughout Kent County, with visits to areas including Warwick, West Warwick, and Coventry.

Key Responsibilities:

Conduct comprehensive annual risk assessments for Medicare Advantage plan members in their homes Review previous and current medical and medication histories with each patient Deliver clear education about existing medical conditions and current medications Identify care gaps and provide documented recommendations for follow-up Focus solely on assessment and educationno responsibility for prescribing medications or developing long-term treatment plans

Qualifications:

Active Nurse Practitioner license in Rhode Island (RI) Board Certification through AANP or ANCC Current Basic Life Support (BLS/CPR) certification Master of Science in Nursing from an accredited institution At least 6 months of relevant Nurse Practitioner experience preferred

Compensation & Perks:
