This per diem opportunity centers on comprehensive evaluations for Medicare Advantage plan members residing throughout Orangeburg County, SCincluding areas such as Orangeburg, Holly Hill, and North. Key Responsibilities:

Conduct annual and post-discharge home health risk assessments in patient residences Evaluate medical history, current conditions, and medication use Offer education to members on chronic disease management, preventive measures, and prescribed therapies Identify gaps in care and present actionable recommendations for follow-upwithout the responsibility of prescribing or creating treatment plans

Qualifications:

Active Nurse Practitioner license in South Carolina Board certification through AANP or ANCC Masters degree in Nursing (MSN) from an accredited institution Current CPR (BLS) certification Minimum of six months recent experience as an NP preferred This role offers compensation of $80$90 per evaluation, with additional mileage reimbursement and administrative time provided.
