Deliver impactful care outside traditional clinical settings as a Nurse Practitioner specializing in home health risk assessments. This per diem position offers the flexibility to build your own schedule, working shifts between Monday and Sunday, with minimum weekly or bi-weekly hourly commitments based on your availability. Youll serve Medicare Advantage plan members throughout Bennettsville and the greater Marlboro County region, including McColl, Clio, and neighboring communities.

Responsibilities:

Conduct thorough annual home health risk assessments for Medicare Advantage members Review and evaluate both current and past medical histories, chronic conditions, and medication usage Educate individuals on their diagnoses and medication regimens to promote understanding and adherence Identify gaps in care, highlighting opportunities for follow-up and recommending next steps for resolution Focus solely on comprehensive assessments; no prescribing or treatment plan creation required

Required Qualifications:

Valid Nurse Practitioner license in South Carolina Board certification via AANP or

ANCC MSN

from an accredited nursing school Current BLS (CPR) certification At least 6 months of nurse practitioner experience preferred Benefit from a competitive rate of $80$90 per completed evaluation, along with a mileage stipend and compensation for administrative time.
