A Home Health Registered Nurse cares for patients in their homes or in community settings, primarily older patients and people with chronic illnesses and limited mobility. Works as part of a community or public health program to coordinate care for patients in between clinical appointments or treatments.
The Occupational Therapist evaluates, plans, and administers occupational therapy services to residents in accordance with physician orders, regulatory requirements, and facility policies. The OT works collaboratively with residents, families, nursing staff, and healthcare professionals to improve functional abilities and promote independence. Sign on Bonus Five Thousand paid out over a year. McCoy Memorial Nursing Home Key Responsibilities Evaluate residents and develop individualized treatment plans. Provide occupational therapy interventions to improve activities of daily living (ADLs), mobility, strength, coordination, and cognitive functioning. Assess adaptive equipment needs and train residents in their use. Document evaluations, treatment plans, progress notes, and discharge summaries accurately and timely. Collaborate with physicians, nursing staff, rehabilitation team members, and family members regarding resident care. Participate in care plan meetings and interdisciplinary team conferences. Ensure compliance with Medicare, Medicaid, state regulations, and facility policies. Educate residents, families, and caregivers on therapy goals and home exercise programs. Maintain productivity standards while delivering high-quality resident-centered care. QualificationsRequired Graduate of an accredited Occupational Therapy program. Current Occupational Therapist license in the state of practice. Current CPR certification (or willingness to obtain). Strong communication, organizational, and documentation skills. Preferred Experience in long-term care, skilled nursing, geriatric rehabilitation, or post-acute care. Knowledge of Medicare Part A and Part B documentation requirements. Experience working with electronic medical records (EMR).
