Flexible shifts available Monday through Sunday Set your own schedule Minimum hourly commitment per week or bi-weekly Responsibilities include: Conducting annual home health risk assessments in accordance with Medicare Advantage requirements Reviewing and evaluating patients medical history, current health status, and medications Educating patients about their medical conditions and treatment plans, ensuring clarity and compliance Identifying potential gaps in care, making recommendations as needed to promote optimal patient outcomes No requirement to create treatment plans or prescribe medications

Desired Qualifications:
