Key responsibilities:

Conduct comprehensive, 45-minute home health risk assessments for Medicare Advantage plan members in Lancaster County and nearby areas (including Lancaster, Indian Land, and Kershaw). Provide clear patient education regarding medical conditions and prescribed medications. Identify gaps in care and make actionable recommendations for follow-up. Required qualifications:

Active Nurse Practitioner license in South Carolina. Current Basic Life Support (CPR/BLS) certification. Board certification through AANP or ANCC.
