Deliver high-quality, patient-centered care as a Nurse Practitioner specializing in home health risk assessments. This per diem position offers flexibility and autonomy, with responsibilities carried out in various locations throughout Chesterfield County, SCincluding Cheraw, Chesterfield, and Pageland. Serving Medicare Advantage members, you will perform thorough evaluations to ensure comprehensive, continuous care.

Core Responsibilities:

Conduct annual home health risk assessments for Medicare Advantage plan members during each visit Evaluate prior and current medical history, chronic medical conditions, and medication regimens Educate patients on their medical conditions and prescribed medications Identify gaps in patient care and provide actionable recommendations for improvement No requirement to develop treatment plans or prescribe medications

Preferred Qualifications:

Active Nurse Practitioner license in South Carolina Board Certification through AANP or ANCC Master of Science in Nursing (MSN) from an accredited institution Current CPR (BLS) certification At least six months of recent, relevant Nurse Practitioner experience preferred This opportunity offers a competitive rate of $80-$90 per evaluation, along with a mileage stipend and compensation for administrative time.
