Seeking a dedicated Nurse Practitioner with home health experience to perform risk assessments for Medicare Advantage plan members across Chesterfield County. This per diem role focuses on providing high-quality, patient-centered care in a flexible, autonomous environment.

Key Responsibilities:

Complete annual home health risk assessments for Medicare Advantage members Visit patients in various Chesterfield County locations Assess medical history, ongoing conditions, and current medications Deliver patient education related to health conditions and prescriptions Identify and address gaps in care, offering recommendations to patients Visits typically last about 45 minutes; may include annual, follow-up, or post-discharge assessments No requirement for developing treatment plans or prescribing medications

Qualifications:

Nurse Practitioner license in South Carolina MSN from an accredited nursing program Board Certification through AANP or ANCC Current CPR (BLS) certification At least 6 months experience as a Nurse Practitioner preferred Proven background in home health assessments is a plus
