Responsibilities:

Perform comprehensive annual and post-discharge health risk assessments in patients homes Review and evaluate prior and current medical and medication histories Educate patients on health conditions, medication regimens, and overall care Identify care gaps, offering recommendations to enhance patient outcomes Focus exclusively on evaluationno diagnosis, treatment planning, or prescribing required

Qualifications:

Active South Carolina Nurse Practitioner license Board Certification (AANP or ANCC) Current Basic Life Support (BLS) certification MSN from an accredited nursing program Preferred minimum of six months NP experience in any clinical setting Enjoy a competitive per-evaluation structure of $80$90, along with mileage stipends and allotted administrative time.
