Seeking an experienced Nurse Practitioner skilled in Home Health for a per diem role performing home-based risk assessments. This opportunity allows you to create your own schedule and make a direct impact by supporting Medicare Advantage members in their homes.

Schedule:

Flexible shifts available Monday through Sunday Option to design your own work schedule Minimum weekly or bi-weekly hourly commitment

Responsibilities:

Conduct comprehensive home health risk assessments for Medicare Advantage plan participants Evaluate and document prior/current medical history, chronic conditions, and detailed medication use Deliver patient education regarding medical conditions and safe medication use Identify gaps in care or follow-up and provide clear recommendations Perform a variety of visit types, such as annual wellness assessments and post-discharge visits Assessments will not require prescribing medication or developing new treatment plans

Qualifications:

Active Nurse Practitioner license in South Carolina Board Certification through AANP or ANCC required Graduation from an accredited MSN program Current CPR certification (BLS) At least 6 months of NP experience is preferred Proven ability to work independently and provide compassionate, patient-centered care Compensation for this position offers $80$90 per completed evaluation, with an additional mileage stipend and coverage for administrative time.
