Flexible shifts available between Monday and Sunday Set your own schedule to fit your lifestyle Minimum weekly or bi-weekly hourly commitments In this role, you will: Visit patients in their homes to conduct comprehensive annual risk assessmentseach typically lasting 45 minutes. Review and document previous and current medical histories, ongoing conditions, and medication regimens. Educate patients about their health, medications, and preventive care strategies. Identify gaps in care and recommend solutions to promote optimal health outcomes. Focus exclusively on assessmentno development of treatment plans or medication prescribing required.

Current Nurse Practitioner license in South Carolina Active CPR (BLS) certification Board Certification through AANP or
