RN Case Manager Home Health Qualifications:

Current RN license in good standing Minimum of 1 year nursing experience Preferred Home Health experience and comprehensive knowledge of OASIS but not required Strong clinical assessment and case management skills Must be able to complete documentation within 24 hours Demonstrated success working independently while taking on increased responsibilities Excellent organization, interpersonal and communication skills Prior experience with electronic medical record documentation and reporting systems

RN Case Manager Home Health Responsibilities:

Direct and coordinate the care provided by a multi-disciplinary care team Deliver exceptional client service and quality clinical care Ensure adherence to policies and procedures Complete Comprehensive assessments, provide direct patient care, client and caregiver teaching Review clinical documentation, including OASIS and care plan coding Act as internal case manager and client advocate Support the medical home model Travel required - limited call required •
