Experienced Nurse Practitioners are sought for a dynamic, per diem home health opportunity focused on conducting risk assessments for Medicare Advantage plan members throughout Lancaster County. The ideal candidate will perform comprehensive in-home evaluations that help close gaps in care and ensure high-quality health outcomes, with the added benefit of setting their own flexible work schedule.

Schedule:

Key Responsibilities:

Conduct in-depth annual health risk assessments during home visits, each lasting about 45 minutes Evaluate members medical history, current conditions, and medication regimens Deliver patient education regarding health conditions and prescribed medications Identify care gaps, providing actionable recommendations for follow-up Visits may include annual assessments or post-discharge check-ins as determined by patient needs No requirement for treatment plan creation or medication prescribing

Qualifications:

Active Nurse Practitioner License in South Carolina Current CPR certification (BLS) National Board Certification by

AANP or ANCC Preferred:

At least 6 months of recent Nurse Practitioner experience MSN from an accredited nursing program
