You will conduct comprehensive home health risk assessments for Medicare Advantage plan members in locations such as Kingstree, Hemingway, Greeleyville, and surrounding areas. Each assessment typically takes about 45 minutes and may include annual health evaluations or post-discharge visits. These assessments are designed to evaluate medical history, conditions, and medications, as well as educate patients and identify care gapsno prescribing or treatment plan creation required.

Key Responsibilities:

Perform annual home health risk assessments for Medicare Advantage members Review and evaluate prior/current medical history, conditions, and medications Educate patients about their health conditions and medication regimens Identify and communicate gaps in care, recommending steps for resolution Complete thorough and accurate documentation for each visit

Qualifications:
