The typical visit lasts about 45 minutes and may involve either an annual assessment or a post-discharge evaluation.

Key Responsibilities:

Complete detailed home health risk assessments for Medicare Advantage members Review and evaluate patients prior and current medical history, ongoing conditions, and medication profiles Educate patients on their conditions and prescribed medications Identify gaps in care and provide actionable clinical recommendations No responsibility for creating treatment plans or prescribing medications

Qualifications:

Active Nurse Practitioner license in South Carolina Current CPR certification (BLS) Board certification through AANP or ANCC A minimum of 6 months of recent Nurse Practitioner experience preferred MSN degree from an accredited school of nursing This role is compensated at $80$90 per evaluation, with an additional mileage stipend and paid administrative time.
