Key Responsibilities:

Complete home-based annual health risk assessments and post-discharge visits for Medicare Advantage beneficiaries Evaluate patients prior and current medical histories, chronic and acute conditions, and medication usage Offer patient education regarding diagnoses and medication regimens Identify care gaps and recommend ways to address them No responsibility for treatment planning or prescribing medications

Qualifications:

Active Nurse Practitioner license in South Carolina MSN degree from an accredited school of nursing Current Board Certification through AANP or

ANCC CPR
