Responsible for functions such as Utilization Review, Discharge Planning, Social Services, and Resource Management. Duties include but are not limited to: Review of patient's medical record, health history, and needs to create a care plan that helps them achieve their goals. Develop, document, and implement plans, which provide appropriate resources to address the social, physical, mental, emotional, spiritual, and supportive needs of the patient. Review criteria to assess and document the appropriateness of admission, continued stay, and level of care Assists patients, their families, and physicians with post-acute placement Advocate for patients ensuring they are receiving the best care in a cost-effective way. Refers cases that do not meet admission and/or continued stay criteria to the designated Physician Educate patients and families about treatment options and health status Performs additional assignments and tasks as assigned by Case Management Department Head. Will be required to be ON CALL at times. Must be able to follow directions and perform work under limited supervision according to department standards when no direction is given. Must be emotionally mature and able to function effectively under stress.
Experience :
1 year of case management and/or utilization review work experience preferred.
Additional Job Description Education:
Graduate from an accredited school of nursing and a minimum of one year of work experience as a Registered Nurse required. Bachelor of Nursing degree preferred.
License :
Licensed as a Registered Nurse by the South Carolina Board of Nursing or a compact state. Case Management certification is preferred.
