Responsibilities:

• Provide comprehensive nursing care to patients in a variety of settings, including hospitals, clinics, and homes.
• Assess patient needs and develop individualized care plans.
• Coordinate and collaborate with healthcare team members to ensure optimal patient outcomes.
• Monitor and evaluate patient progress, adjusting care plans as necessary.
• Educate patients and their families on disease management, treatment options, and preventive care.
• Utilize case management skills to coordinate healthcare services and resources for patients.
• Conduct utilization management activities to ensure appropriate use of healthcare resources.
• Document patient information accurately and timely in electronic health records.

Qualifications:

• Valid Registered Nurse (RN) license in the state of GA/SC.
• Minimum of 2 years of nursing experience, preferably in Hospice.
• Knowledge of pediatric nursing principles and practices.
• Strong patient service orientation with excellent communication skills.
• Solid understanding of human anatomy, physiology, and pathophysiology.
• Experience in case management or utilization management is highly desirable.
• Familiarity with critical care and ICU settings is a plus.
• Ability to assess vital signs and interpret diagnostic test results.
• Proficient in utilizing electronic health record systems.
• Strong organizational skills with the ability to prioritize and multitask effectively.
