• $2,600 Weekly Requirements Active Registered Nurse (RN) License 2+ years of Home Health RN experience required Homecare Homebase (HCHB) experience required Current BLS Certification Valid driver's license and reliable transportation (if required by the facility) Responsibilities Provide skilled nursing care to patients in their homes in accordance with physician orders and care plans.

Perform comprehensive patient assessments and develop individualized care plans. Administer medications, treatments, and wound care as prescribed. Educate patients and families regarding disease management, medications, and home care needs. Monitor patient progress and communicate changes in condition to physicians and interdisciplinary team members. Complete accurate and timely documentation using Homecare Homebase (HCHB) . Coordinate care with therapists, social workers, physicians, and other healthcare professionals. Ensure compliance with agency policies, state regulations, and quality standards.
