As a Hospice RN Case Manager, you will serve as a vital link in delivering personalized, patient-centered care that emphasizes comfort, dignity, and quality of life. Wren Hospice is locally owned and operated. We are offering a $5000 sign on bonus for full time ideal candidates that possess strong case management skills, hospice experience preferred. Responsibilities Conduct thorough patient assessments to determine eligibility and develop individualized care plans aligned with hospice standards. Coordinate interdisciplinary team efforts to ensure seamless delivery of medical, emotional, and spiritual support tailored to each patient's needs. Manage symptom control and palliative interventions by utilizing your expertise in physiology, infectious disease care, and acute care practices. Facilitate effective discharge planning and transition of care from hospital or inpatient settings to home or outpatient hospice services. Oversee medication management, including pain control and symptom relief, ensuring adherence to best practices and safety protocols. Collaborate with primary care providers, specialists, and community resources to optimize patient outcomes and maintain compliance with HIPAA regulations. Provide education to patients and families about disease progression, treatment options, and advance directives with clarity and compassion. Document all patient interactions accurately within medical records systems while maintaining confidentiality in accordance with HIPAA standards. Requirements Valid RN license with current registration in the state of practice. Graduate of an accredited school of nursing. One (1) to two (2) years of recent acute care experience in an institutional setting, and/or one (1) to two (2) years of recent experience in home care or hospice preferred. CPR certification and valid driver's license with automobile insurance. Bachelor's or Associates degree, with one (1) year of hospice care experience preferred. Must exhibit the ability to interact well with a great diversity of individuals and the ability to organize and execute Wren Hospice policies and procedures. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
