Job Overview We are seeking a dedicated full time and part time On-Call Nurse - RN to join our compassionate team. This role offers flexible, on-demand nursing support to ensure seamless patient care during after-hours or when regular staff are unavailable. The ideal candidate will possess a broad skill set , with experience working in diverse healthcare settings, including hospice, hospital, and long-term care environments. This position emphasizes patient-centered care, clinical excellence, and effective communication. Responsibilities Provide comprehensive nursing assessments and interventions for patients in various settings, including hospice, home care, assisted living, and inpatient facilities. Respond promptly to urgent patient needs during on-call hours, including managing acute pain. Collaborate with interdisciplinary teams to develop and update individualized care plans aligned with patient goals and clinical standards. Maintaining HIPAA compliance. Assist with triage by evaluating symptoms, prioritizing care needs, and coordinating emergency responses when necessary. Educate patients and families about disease management, medication protocols, and advanced directives to promote informed decision-making. Qualifications Valid RN license with current registration; additional certifications in Basic Life Support (BLS) required. Familiarity with MatrixCare for accurate documentation. Strong assessment skills with the ability to evaluate vital signs, and perform physical examinations. Bachelor's or Associates degree, with one (1) year of hospice care experience preferred. Must exhibit the ability to interact well with a great diversity of individuals and the ability to organize and execute Wren Hospice policies and procedures. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
