This role is responsible for assessing, treating, monitoring, and documenting wounds while collaborating with the interdisciplinary team to promote healing, prevent skin breakdown, and improve overall resident outcomes. Key Responsibilities Perform comprehensive skin and wound assessments Develop, implement, and evaluate individualized wound care treatment plans Monitor and document wound healing progress and treatment effectiveness Perform wound care treatments and dressing changes as ordered Identify residents at risk for skin breakdown and implement preventative interventions Collaborate with physicians and interdisciplinary team members regarding wound care needs Ensure accurate and timely documentation in accordance with facility policies and regulatory requirements Educate nursing staff on wound prevention, skin integrity, and treatment protocols Participate in care plan meetings and resident assessments Monitor infection prevention practices related to wound care Maintain wound care supplies and treatment records Assist with quality improvement initiatives related to skin and wound management Qualifications Current, unrestricted Registered Nurse (RN) license in South Carolina Current CPR certification Experience with wound care preferred Experience in skilled nursing or long-term care preferred Experience with PointClickCare (PCC) preferred Strong assessment, documentation, and clinical decision-making skills Excellent communication and teamwork abilities Requirements Must maintain all required continuing education and licensure requirements Must remain in good standing with the South Carolina Board of Nursing Commitment to providing compassionate, resident-centered care What Makes You a Great Fit Passion for wound care and improving resident outcomes Detail-oriented with strong clinical judgment Compassionate and resident-focused Strong organizational and communication skills Team player who enjoys collaborating with others We are an equal opportunity employer.
