Core Responsibilities:

Complete annual risk assessments in members homes, with each visit typically lasting 45 minutes Review patients medical and medication histories, current conditions, and prior health events Deliver education on diagnoses, medications, and overall health management Identify care gaps, sharing recommendations for improvement with patients No requirements for prescribing medications or creating treatment plansfocus is on assessment and education

Desired Qualifications:

Active Nurse Practitioner license in the state of South Carolina Current BLS (CPR) certification Certification from the AANP or

ANCC MSN

degree from an accredited nursing program Minimum of six months of nurse practitioner experience preferred
