Columbia, SC (Richland County Coverage Area)

40 Hours/Week Overview We are seeking a compassionate and experienced Registered Nurse (RN) Home Health Case Manager to provide high-quality, patient-centered care throughout Richland County, South Carolina. This role offers autonomy in scheduling, opportunities to build meaningful patient relationships, and the ability to make a lasting impact by helping patients remain safely in their homes. Schedule Full-time, 40 hours per week Agency office hours: Monday through Friday, 8:00 AM to 4:30 PM Key Responsibilities Complete admissions and start-of-care assessments for home health patients Develop, implement, and manage individualized care plans Provide ongoing case management for assigned patients Conduct comprehensive nursing assessments and reassessments Coordinate care with physicians, therapists, caregivers, and interdisciplinary team members Provide clinical oversight and guidance to Licensed Practical Nurses (LPNs) Ensure accurate and timely documentation in compliance with regulatory and agency requirements Complete OASIS assessments and maintain quality standards Educate patients and families regarding diagnoses, treatment plans, medications, and disease management Assess, monitor, and guide wound care treatment as needed Monitor patient progress and adjust care plans accordingly Required Qualifications Active and unrestricted Registered Nurse (RN) license Current CPR Certification Home Health experience
