Key Responsibilities:

Perform annual home health risk assessments for Medicare Advantage health plan members in their homes Review and evaluate previous and current medical history, conditions, and medications Educate patients on their existing health conditions and medication regimens Identify care gaps and offer guidance for appropriate follow-up, ensuring comprehensive patient understanding No ongoing treatment planning or prescription responsibilities

Qualifications:
