Key Responsibilities:

Conduct comprehensive home health risk assessments, each taking approximately 45 minutes Evaluate patients prior and current medical history, ongoing conditions, and medication regimens Provide personalized education to patients about their health conditions and treatments Identify care gaps and recommend solutions to enhance patient wellbeing No treatment planning or medication prescribing required

Qualifications:
