A skilled Nurse Practitioner is needed to conduct home health risk assessments for Medicare Advantage plan members in various locations within Haywood County. Key Responsibilities:

Perform comprehensive home health risk assessments for Medicare Advantage members Evaluate patient medical histories, ongoing conditions, and current medication usage Educate patients on their medical conditions and medications to encourage understanding and compliance Identify gaps in care and provide clear, actionable recommendations for follow-up Visits may include annual health assessments or post-discharge evaluations; each visit averages 45 minutes No treatment planning or prescribing medications required

Qualifications:

Active Nurse Practitioner license in Tennessee Current CPR certification (BLS required) Board Certification from AANP or

ANCC MSN

degree from an accredited nursing program At least 6 months of recent experience as a Nurse Practitioner preferred
