PRN LTAC RN

to deliver high-acuity, patient-centered care in a long-term acute care setting. In this per diem role, you'll perform comprehensive assessments, manage complex treatments (including ventilators, wound care, and IV therapies), and coordinate with an interdisciplinary team to support optimal outcomes and safe transitions of care. Responsibilities Provide high-acuity nursing care to LTAC patients per plan of care Perform ongoing assessments, monitor vitals, and respond to changes in condition Administer medications, manage IVs, and perform complex treatments and wound care Collaborate with physicians and interdisciplinary team on care plans and discharge goals Maintain accurate, timely documentation in EMR and adhere to all policies and regulations Educate patients and families on treatment plans and ongoing care needs Uphold safety, infection control, and quality standards in all clinical activities Required Skills LTAC nursing care High-acuity patient assessment Ventilator management Wound care IV therapy and medication administration Care planning and documentation (EMR) Infection control Patient and family education Interdisciplinary team collaboration Time management and prioritization
