Key Responsibilities:

Conduct comprehensive, 45-minute home health risk assessments for Medicare Advantage members Review and evaluate medical history, current medical conditions, and medication records Educate patients regarding diagnoses, medications, and self-care practices Identify care gaps and provide actionable recommendations to improve patient outcomes Perform visits for annual health assessments or post-discharge follow-ups No requirement to create treatment plans or prescribe medications

Qualifications:

Current Nurse Practitioner license in Tennessee Active CPR (BLS) certification Board Certification via AANP or ANCC At least 6 months of recent Nurse Practitioner experience preferred Master of Science in Nursing (MSN) from an accredited institution
