Key Responsibilities:

Conduct home health risk assessments, each typically lasting 45 minutes, with visits ranging from annual assessments to post-discharge follow-ups Review patients prior and current medical history, conditions, and medication use Educate patients about their health conditions and prescribed medications Identify and address any gaps in care, offering actionable recommendations No requirement to create treatment plans or prescribe medications

Preferred Qualifications:
