Qualifications RN License Full Job Description Role:
Registered Nurse providing home-based nursing care in compliance with laws, regulations, and agency policies.Responsibilities:
Deliver high-quality clinical services within scope of practice and infection control standards Coordinate care with healthcare team from admission to discharge Conduct clinical nursing assessments per program and payer requirements Verify patient eligibility and medical necessity for services Develop and revise individualized care plans with community providers Ensure care plan appropriateness and obtain physician approval for revisionsQualifications & Skills:
Current and unrestricted RN licensure in the practice state Valid driver's license, vehicle insurance, and access to reliable transportation CPR certification or ability to obtain within 90 days Ability to work effectively in diverse home environmentsPreferred:
1+ years RN experience, independence, strong communication, organizational skillsKey Attributes:
