PRN LTAC RN

to deliver high-quality, complex nursing care in a long-term acute care setting. In this per diem role, you'll manage high-acuity patients requiring ventilator support, complex wound care, and intensive monitoring. Responsibilities include comprehensive assessments, medication administration, IV and central line care, and close collaboration with an interdisciplinary team. Responsibilities Provide direct nursing care to medically complex LTAC patients on a PRN basis Perform comprehensive assessments and develop individualized care plans Administer medications, manage IVs, and monitor telemetry and vital signs Manage ventilators, tracheostomies, and complex wounds per facility protocols Collaborate with physicians, therapists, and case managers to coordinate care Document accurately in the EHR and ensure compliance with regulatory standards Educate patients and families on treatment plans and discharge goals Maintain infection control and patient safety practices at all times Required Skills Long-term acute care nursing Patient assessment and care planning Ventilator and trach management IV therapy and medication administration Central line care Wound and skin integrity management Telemetry and hemodynamic monitoring Infection prevention and control Electronic Health Record (EHR) documentation Interdisciplinary team collaboration
