Primary Responsibilities:

Complete comprehensive home health risk assessments for Medicare Advantage plan members Review and evaluate prior/current medical history, medical conditions, and medications Provide clear education regarding medical conditions and medications to patients Identify any care gaps and recommend steps to address them Visits may include annual health assessments or post-discharge evaluations (approximately 45 minutes each) No need to prescribe medications or develop treatment plans

Preferred Qualifications:

Unrestricted Nurse Practitioner (NP) license in Tennessee Board certification via AANP or ANCC Active CPR certification (BLS) Minimum of six months experience as a Nurse Practitioner is preferred Master of Science in Nursing (MSN) from an accredited program In this position, youll earn $80$90 per completed evaluation, along with a mileage stipend and compensation for administrative time.
