PRN LTAC RN

to deliver high-quality, complex patient care in a long-term acute care setting. You'll manage ventilator-dependent, post-ICU, and medically complex patients, perform thorough assessments, administer meds and treatments, and coordinate interdisciplinary care. Responsibilities Provide direct nursing care to LTAC patients including complex med-surg, post-ICU, and ventilator-dependent patients Administer medications, perform treatments, and monitor patient status, documenting accurately in the medical record Collaborate with physicians, therapists, and interdisciplinary team to develop and update care plans Perform and monitor wound care, trach and ventilator management, and tube feedings per facility protocols Respond to patient changes in condition, perform assessments, and implement appropriate interventions Educate patients and families on care plans, medications, and discharge needs Ensure compliance with infection control, safety, and LTAC regulatory standards Float as needed within facility based on acuity and staffing needs, maintaining flexibility for PRN shifts Required Skills Long-term acute care (LTAC) nursing High-acuity patient assessment Ventilator and tracheostomy management Wound and skin care management IV therapy and central line care Medication administration and titration Telemetry and hemodynamic monitoring Care planning and care coordination Infection prevention and control Electronic Medical Record (EMR) documentation
