Job Description
Overview We are seeking a highly motivated and detail-oriented MDS (Minimum Data Set) Coordinator to join our dynamic healthcare team. In this vital role, you will oversee the accurate and timely completion of MDS assessments, ensuring compliance with federal and state regulations, including Medicare and NCQA standards. Your expertise will directly impact patient care quality, reimbursement processes, and clinical documentation improvement initiatives. Duties Coordinate and facilitate comprehensive MDS assessments for inpatient, outpatient, nursing home, hospice, and home care settings, ensuring adherence to all regulatory guidelines. Review medical records, clinical documentation, and coding (ICD-9, ICD-10, CPT) to support accurate case mix index (CMI) calculations and reimbursement. Collaborate with interdisciplinary teams—including nursing, medical management, case management, and physicians—to gather necessary information for accurate MDS completion. Ensure compliance with HIPAA regulations by maintaining confidentiality of patient information during documentation review and data entry. Utilize electronic health record (EHR) systems such as Epic, Cerner, Athenahealth, eClinicalWorks, or similar platforms to document assessments efficiently. Conduct utilization review and utilization management activities to optimize patient care plans while adhering to Medicare and NCQA standards. Participate in clinical documentation improvement initiatives to enhance the accuracy of medical records and coding accuracy. Stay current with changes in healthcare regulations related to acute care, critical care, ICU, PICU, trauma centers (Level I & II), emergency medicine, primary care, pediatrics, and other specialties. Support discharge planning processes by providing precise data to facilitate smooth transitions of care across inpatient and outpatient settings. Qualifications Proven experience with Epic EMR systems or similar EHR platforms; familiarity with Cerner is a plus. Strong knowledge of medical terminology, anatomy, physiology, and clinical documentation standards. Experience in managed care environments with understanding of Medicare policies and DRG (Diagnosis-Related Group) coding procedures. Demonstrated expertise in
ICD-9/ICD-10
coding, CPT coding, and DRG assignment for accurate reimbursement. Background in hospital settings such as Level I trauma centers or Level II trauma centers is highly desirable; experience in nursing homes or hospice care is a plus. Knowledge of healthcare regulations including HIPAA compliance and NCQA standards for quality assurance. Excellent organizational skills with the ability to review complex medical records efficiently while maintaining attention to detail. Strong communication skills to collaborate effectively with multidisciplinary teams across inpatient and outpatient environments. Prior experience in case management or utilization review roles within acute care or critical care settings preferred.
