Responsibilities include:
Provide skilled nursing visits in the home setting according to physician, nurse practitioner, or physician assistant orders Perform patient assessments and develop nursing care plans Educate patients and families on medications, disease management, and recovery goals Monitor patient progress and report changes in condition to providers Document all patient care using the electronic medical record (EMR) Collaborate with therapists, aides, and case managers to support patient outcomes Participate in team meetings and patient care conferences Participate in the on-call rotation ________________________________________ Registered Nurse (RN)
- Home Health Qualifications Required: Active Registered Nurse (RN) license
- Tennessee Current CPR certification Valid driver's license Reliable transportation and current auto insurance
