PRN LTAC RN

to provide high-acuity nursing care in a long-term acute care setting. In this flexible, per diem role, you'll manage complex, medically fragile patients, including ventilator, trach, and wound care, continuous assessments, and coordinated care planning. You'll document in EMRs, collaborate with an interdisciplinary team, and educate patients and families. Responsibilities Deliver high-quality nursing care to complex LTAC patients in a PRN capacity Monitor, assess, and document patient status and respond to changes in condition Administer medications, treatments, and perform ventilator, trach, and wound care Collaborate with physicians and interdisciplinary team to develop and update care plans Educate patients and families on treatment plans and discharge goals Ensure compliance with facility policies, safety protocols, and regulatory standards Use EMR systems accurately for timely documentation and care coordination Support a culture of respect, integrity, and excellence in patient care Required Skills Long-term acute care nursing High-acuity patient management Ventilator and trach care Wound care management Telemetry monitoring IV medication administration Care planning and coordination Patient and family education Electronic Medical Record (EMR) documentation Interdisciplinary team collaboration
