RN Case Manager - Home Health

We are seeking a dedicated and compassionate RN Case Manager to join our team. An ideal candidate will work closely with our patients, their families, and fellow healthcare professionals to deliver high-quality care that promotes optimal health outcomes.

Responsibilities:

• Conduct comprehensive nursing assessments
• Coordinate patient care/documentation to obtain/maintain approvals for care
• Develop/draft and implement individualized care plans
• Coordinate all aspects of patient care
• Provide patient care in the home setting as needed
• Provide education in regards to patient's care to anyone applicable
• Adhere to HIPAA guidelines.
• Effectively communicate with outside healthcare providers as well as the care team.
• Be involved in communications with families, patients and physicians
• Provide community resources as needed
• May be required to perform other duties as needed/assigned
• Travel to patient homes and physician appts required

Requirements:

• Current registered nurse license in good standing
• Experience in home health/hospice setting
• Previous case management experience preferred
• Good organizational and time management skills
• Working knowledge of computer and internet-based applications

Pay:

$42

• 48 an hour
